Hera Hub Carlsbad Wellness Room Rental Agreement

By reserving the Wellness Room at Hera Hub Carlsbad, I agree to all of the following by 1) checking the box next to every term below and 2) adding my initials to the left of each term:

I understand that my reservation of the Wellness Room must include setup, time with my client, and post-treatment cleanup.

I will bring all pillows, linens and towels needed to maintain the cleanliness of the massage table, face cradle, and the Wellness Room generally.

I confirm that I will ensure that the massage table is securely set up, including that the legs are in a locked position, prior to my client's arrival.

I will clean up after myself and my client, leaving the massage table and Wellness Room in the same condition that they were in when I arrived.

I will promptly repair any damage to the massage table or Wellness Room caused by myself or my clients.

I will not use the Wellness Room, or permit it to be used, in any manner that violates any law, statute, ordinance or regulation.

I agree to hold Hera Hub Carlsbad and its staff harmless from any and all loss, claims, damage, and liability to any person or property occurring in the Wellness Room or at Hera Hub Carlsbad from any cause whatsoever.

I maintain current general liability insurance, including bodily injury, personal injury and property damage coverage for my wellness business, with a limit for claims arising out of any one occurrence of at least $1,000,000. I have named “Hera Hub Carlsbad” as an “additional insured” on my policy.

Please attach proof of insurance and additional insured endorsement for Hera Hub Carlsbad at least 1 business day prior to your first reservation in the Wellness Room.
